Health Care Directive Checklist

Follow the steps below to help ensure that your Health Care Directive is effective.
1. Complete your HCD
- Complete LawDepot’s Health Care Directive Questionnaire to your satisfaction.
- If you are satisfied with your HCD, you can move onto step two. If you are not satisfied, you can use the LawDepot Editor to make any changes you wish to your HCD.
2. Have your HCD professionally reviewed (Optional)
Once you have completed your HCD, you may wish to have it reviewed for errors. Specially trained LawDepot staff can provide a Peace of Mind Review within two business days.
3. Print your HCD
You should print several copies of your HCD:
- One for you
- One for your physician,
- One for each of your Health Care Representatives,
- Possibly a few extra copies to give to family or friends, or to store as extras.
Note: If you have purchased a LawDepot Peace of Mind Review, you should wait for your review to be completed before printing, as your review may result in additional changes you wish to make.
4. Execute your HCD
Sign all of the copies of your HCD that you have printed. View Signing Details
5. Inform your Health Care Representative
Inform your Health Care Representative that you have appointed him or her as your Health Care Representative.
6. Inform your Alternate Health Care Representative (If Applicable)
Inform your Alternate Health Care Representative that you have appointed him or her as your Alternate Health Care Representative.
7. Distribute your HCD
- Store your signed copy or copies in a secure place.
- Send your Health Care Representative(s) a signed copy of your HCD.
- Send your primary physician or family doctor a signed copy of your HCD.
- Send out any additional copies that you wish to distribute to family or friends.
8. Inform your family and friends (Optional)
Other than your Representative(s), you are not required to inform any family or friends about your HCD. However, the more people who know you have created a HCD, the more likely it is to be followed.
9. Review your Health Care Directive from time to time
You should review your Health Care Directive from time to time to ensure that it reflects your wishes at all times. Should you decide in the future that your HCD no longer reflects your wishes, you can Revoke your HCD.
